Along with the students of Perris Elementary School District, our employees are our most valuable asset. The Risk Management Department is dedicated to keeping the employees of Perris Elementary School District safe, but accidents do happen. When an accident occurs and an employee becomes injured on the job, it is important to follow the correct process to manage their claim appropriately so they receive the most efficient and consistent care possible. The Risk Management Department's goal is to support employees' healing and help them return to work as quickly as possible.
What do I do if I get injured at work?
Report your injury to your supervisor immediately, but no longer than 24 hours after your accident!
Call S1 Medical at 833-691-9022. You will be triaged and provided with a referral for medical treatment if needed. S1 Medical is available 24 hours per day/ 7 days per week
Complete the Claim Packet and return it to your Supervisor. Your supervisor will forward the information to Risk Management.
Work Injury/ Illness Claim Packet
Pre-Designation of Personal Physician Form - You may be able to choose the doctor who will treat you for a job injury or illness. If eligible, you must tell your employer, in writing, the name and address of your personal physician or medical group before you are injured. You must obtain an agreement from your physician to treat you for your work injury.

For emergency care, an employee should go to the nearest healthcare provider. Once their condition is stable, they will be directed to a MPN provider for continued care.
